Bracknell Forest Health and Wellbeing Service

The Health and Wellbeing Scheme (Exercise Referral) in Bracknell is a 6 month programme to support residents start or increase their physical activity while living with a long-term health condition.

At the initial consultation, a qualified exerciser referral fitness instructor specialising in long-term health condition will take baseline measurement and ask participants to complete a questionnaire. These are repeated at 3 and 6 months to help monitor progress. During this initial consultation, the instructor will also discuss goals, exercise preferences and the best way to support you.

The service is free to people who can show proof of qualifying benefits, otherwise the service is offered at a reduced rate of £32.23 per month.

Residents can be referred by their GP, Social Prescriber, Health Care Professional, School Nurse, Adult Service and other community partners.
